Diabetes and your eyes

Diabetes damages the small blood vessels of the retina long before it affects how well you see. Vessels leak, swell, and eventually close off, and the retina responds by growing fragile new vessels that bleed. By the time diabetic retinopathy affects central vision, significant damage has usually already happened, which is exactly why the annual dilated exam matters so much even when your vision feels perfect.

A diabetic eye exam involves dilating the pupils so we can examine the entire retina, not just the central portion, along with retinal imaging that documents the state of your vessels for comparison next year. We look for microaneurysms, hemorrhages, leakage, swelling at the macula, and any early vessel growth. We send a report to your primary care physician or endocrinologist so your eye findings are part of your overall diabetes management, and we tell you honestly what we see and how urgently it needs attention.

Urgent and same-day eye problems

A red, painful, or suddenly blurry eye belongs in an optometrist's chair, not an urgent care waiting room. We have the microscope, the dyes, the instruments, and the training to diagnose and treat it properly, usually in a single visit.

• Red eye, bacterial, viral, and allergic conjunctivitis
• Corneal abrasions and foreign body removal, including metal and rust rings
• Styes, chalazia, and lid infections
• Contact lens–related infections and corneal ulcers
• Sudden floaters, flashes of light, or a curtain in the vision
• Eye pain, light sensitivity, and inflammation inside the eye

Other systemic conditions we monitor

High blood pressure leaves signs in the retinal vessels. High cholesterol can show up as deposits in the cornea or as a vessel blockage. Thyroid disease changes the position and comfort of the eyes. Autoimmune conditions inflame the surface and sometimes the inside of the eye. Several common medications, hydroxychloroquine most notably, require regular retinal monitoring to catch toxicity before it affects vision.

If you're on a medication that requires eye monitoring, tell us at your visit. We'll set up the right baseline testing and the right interval, and coordinate results with the prescribing physician.

Surgical co-management

When a condition needs a surgeon, cataract surgery, a retinal problem, advanced glaucoma, we handle the referral and stay involved. That means pre-operative measurements and counseling so you know what to expect, coordination with the surgeon's office, and post-operative visits close to home instead of a long drive for every follow-up.

One important billing note: medical eye visits such as infections, foreign body removal, dry eye disease, and diabetes monitoring are not covered by vision plans. At our office we accept Medicare PPO for medical visits; otherwise these visits are paid out of pocket. We'll always explain the cost before we treat.
